Overview

Sore throats are one of the top reasons people visit a doctor — and one of the top reasons antibiotics are unnecessarily prescribed. The vast majority of sore throats are caused by viruses, resolve within a week without antibiotics, and need only paracetamol, fluids, and rest. Bacterial tonsillitis from Streptococcus pyogenes (strep throat) does need antibiotics — not primarily because antibiotics shorten the illness but because untreated strep can cause rheumatic fever, a serious condition that damages the heart. Distinguishing viral from bacterial tonsillitis clinically is difficult; a rapid strep test or throat swab removes the guesswork. Recurrent tonsillitis, peritonsillar abscess, and tonsillar obstruction are the situations where an ENT opinion — and potentially tonsillectomy — becomes relevant.
